9th Grade/New Enters

Physical Exam submitted to the Health Office dated 9/4/2024 or newer:

• Upon entering 9th grade

Completed screenings:

• Scoliosis 

• Color vision (for new enters only)

Immunization proof submitted to the Health Office:

•  Upon entering 9th Grade/New enters

11th Grade

Physical Exam submitted to the Health Office dated 9/4/2024 or newer:

• Upon entering 11th grade

Completed screenings:

• Near vision 

• Distance vision 

• Hearing

12th Grade

Immunization proof submitted to the Health Office upon entering 12 grade:

• Meningococcal A (2 valid doses or 1 if the dose was received at 16 years or older)

Working Papers

• Physical exam dated a year from the date of the working paper application (or newer)

• During the summer, bring a copy of the physical exam to the RCK main office

Sports

• Must have a valid physical exam, dated within a year of the start date of the sport. See Athletic Registration Information for more details.

Effective July 1, 2018 – New York State Education Law requires new entrants to the Wappingers Central School District and each student in grades Kindergarten, 1, 3, 5, 7, 9 and 11 to have a physical examination on file. 

Also mandated by New York State are health screenings. If your doctor completes any of the state-mandated screenings on the physical exam form, then they will not be repeated at school.

Effective January 1, 2021 - all physical exams for public school students must be documented on the NYS Required Health Examination form. A "Visit Summary" is not a physical exam and will not be counted as one for the school year.  A copy of this form is located on the bottom of the page for your convenience. 

**Please Note: All physical exams must be completed on the NYS form. If a physical exam is not completed on the appropriate form, then the WCSD Medical director or one of his associates  must repeat a physical at a later date. **

A Dental Health Certificate is recommended to be on file for 9th and 11th grade. However, it is not mandated to be on file.
